On June 13, 2017, the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors levied a civil penalty of $3500 and costs of investigation of $684.25, placed a public reprimand on the permanent disciplinary record with the Board, indefinitely suspended for a minimum period of six (6) months, and required completion of at least thirty (30) hours of remedial education by Alexis Anna-May Koutroulakis, license no.: CW016332, of Philadelphia, Philadelphia County, because she departed from or failed to conform to the standards of acceptable and prevailing practice by having a personal, social, or romantic relationship with a current client; engaged in an improper dual relationship by having a personal, social and romantic relationship with a current client; engaged in sexual intimacies with a current client; and departed from the standards of professional practice and conduct because she violated the Clinical Social Work Association Code of Ethics.
