American HomePatient (AHP) discovered on January 7, 2017 that several computer hard drives had been stolen from its Newark, Delaware office. The theft is believed to have occurred on or about January 6, 2017. The hard drives contained certain individual information, which may have included names, addresses, AHP account numbers, social security numbers, diagnosis codes, date of birth, insurance information, and treatment information for some individuals. Upon discovery, AHP immediately notified local law enforcement. In response to this incident, and to help prevent an incident like this from happening in the future, AHP is implementing new physical and technological safeguards in its office and on its systems that contain personal information. AHP has arranged a dedicated call center run by a third party, Epiq, to assist in identifying the affected individuals, offering individuals credit monitoring and identity theft protection services at no cost for one year, as well as providing individuals with information on how to mitigate the chances of fraud or identity theft.
